Q: I have seen a big change in my elderly parents' ability to take care of themselves. I'm not sure how serious the situation is but I know they need help. Can you help me?

A: Yes. With our Geriatric Care Management service, a care manager first conducts an assessment of your parents in their home environment. This assessment includes a safety review. The manager identifies any problems, finds community resources to meet their needs and develops a care plan.

If you wish, your care manager can provide ongoing care management. Ongoing care management can include implementing the care plan, coordinating services and monitoring the situation to see that any emerging needs are addressed. A typical case takes 4 - 10 hours of a care manager's time, billed at an hourly rate.

 

Q: I know what type of care my loved one needs, but I need help finding a specific provider in my area. I don't have time to do research or make phone calls. Can you help me?

A: Our consultation service can provide you with carefully researched options, screened according to availability and your specific needs and preferences. Typically, this service will take 2-4 hours of a care manager's time, billed at an hourly rate.

 

Q: Why won't you just give me a list of assisted living facilities in my area?

A: If we just gave you a list, you'd still have much work ahead of you. Instead, we develop a customized report with options that have been specifically screened according to your loved one's needs, resources and eligibility requirements. We communicate with each facility in the development of your report, so it is current and only has facilities with available spots.

 

Q: Not sure I can afford to pay for this. I can do my own research and make phone calls. How will this service help me?

A: Our care managers have years of experience researching community resources, and interpreting the needs of seniors. For just a few hours of a care manager's time, you will tap into a wealth of knowledge and expertise that can benefit your loved one and make you feel confident you are making good, informed decisions about his care.

 

Q: I work full time and cannot always be there when Mom needs me. Can you help?

A: Yes. With our Geriatric Care Management service, your care manager can be your eyes and ears to help you manage your relative's care. For example, we can accompany your mother to the doctor and communicate to you what occurred.

 

Q: My loved one lives in another state, can you help me?

A: Yes.

 

Q: What qualifications do your care management staff have?

A: Our staff has years of experience addressing the needs of the elderly and their family caregivers, researching community resources, working with the elderly and their families and the issues that surround these dynamics, developing specialized care plans, and educating families so that they are empowered to provide the best care for their loved ones. Staff is certified by the National Academy of Certified Care Managers.
